Print a copy of the Maxtel application and fax or mail it to Maxtel. A signature is required on the application along with a photocopy of both sides of your credit card.

We do ask that payments for monthly usage be made by credit card. You can use American Express, Visa, MasterCard, Diners Club or JCB. We'll activate your account as soon as preliminary credit card verification is complete. We'll send you an email that (1) confirms your account has been activated and (2) contains your Personal Access Number to trigger callbacks.

Authorization Upon Submitting Application:
I authorize MaxTel to charge my credit card each week for the amount of my MaxTel bill plus any outstanding balance due at the end of the billing cycle. I am personally responsible for any and all of my MaxTel charges that are not covered by this authorization (e.g., credit card becomes invalid). This authorization is to remain in effect until MaxTel receives written notification from me to cancel this authority.
